Who qualifies
All GLP-1 medicines sold in the US require a prescription. For an approved use shown on the label, a prescriber provides a prescription:
- Heart disease and obesity or overweight in adults: this cuts the risk of heart attack and stroke, and of heart-related death (injection and tablets).
- This covers losing weight and keeping the weight off long term, together with diet and activity. It applies to adults and children 12 and older who have obesity, and to adults who have overweight plus a weight-related condition.
- In adults, this injection treats MASH liver disease that has scarring of moderate to advanced severity (no cirrhosis).

Scam signals
- A seller that needs no prescription is not licensed as a US pharmacy.
- A brand price far below what the manufacturer charges: the lowest self-pay price for the brand is $149 a month.
- Because the FDA has not approved compounded drugs, those sold as the brand are neither Ozempic, Wegovy, Mounjaro nor Zepbound products.
- Salt versions that differ: semaglutide sodium and semaglutide acetate are not what the approved drugs use as their active ingredient.
- Claiming it matches the brand: the FDA sent 30 warning letters about misleading claims to telehealth companies.
- Harm reports: the FDA has 990 adverse event reports on file that are tied to compounded semaglutide.
- A monthly price that leaves out the drug: the FTC acted against one program over this; read the NextMed review.
