Prior authorization: the rules
- Medicaid programs in states across the country typically require prior authorization for GLP-1s
- For diabetes, required records include your diagnosis and a recent A1C. If weight loss coverage applies, your measured BMI and weight-related conditions are included in the records.
- Your prescriber files the request with your Medicaid plan; your prescriber is the one who sends it.

How to check your plan
- Look for the drug on the state's list: Kentucky Medicaid drug list.
- If a managed care plan runs your Medicaid, call the member number on your card and ask whether the drug requires prior authorization.
- Find out from your prescriber which GLP-1 option is preferred, because drugs with preferred status are approved faster.
- The notice you get for an approval or denial lists the rule that applies to you.
