Who qualifies
In the US, every GLP-1 sold needs a prescription. A prescriber writes it for a use approved on the label:
- Adults with heart disease, plus obesity or overweight, see a lower chance of stroke, heart attack, or death tied to the heart. Both injection and tablets work this way.
- Used alongside diet and activity, it supports weight loss and helps maintain weight loss over the long term. This applies to adults and children 12 and older with obesity, along with adults who have overweight and a weight-related condition.
- Adults are treated with the injection for MASH liver disease when scarring is moderate to advanced, without cirrhosis.

Scam signals
- Warning sign: no prescription required. A seller who skips it cannot be a licensed US pharmacy.
- For the brand, the lowest self-pay price is $149 a month, far below the manufacturer's own price.
- Sold as the brand but compounded: such drugs lack FDA approval, so they are not Ozempic, Wegovy, Mounjaro, Zepbound.
- Salt forms: approved drugs use an active ingredient unlike either semaglutide sodium or semaglutide acetate.
- Claiming it is identical to the brand: the FDA sent 30 warning letters to telehealth companies over misleading claims.
- Harm reports: FDA has received 990 reports of adverse events connected to compounded semaglutide.
- The drug was left out of the monthly price, so the FTC acted against one program; see the NextMed review.
