Prior authorization rules
- Required: prior authorization. It is required
- Your BMI qualifies if it is 30 or higher. A BMI of 27 also qualifies if you have a weight-related condition
- For diabetes, your records include a recent A1C result and a diagnosis for you. For covered weight loss, the factors are your measured BMI plus weight-related conditions.
- Your prescriber sends the request to your Medicaid plan.

How to check your plan
- Use the Minnesota Medicaid drug list to find the drug.
- Your card lists a member number. Call it and ask whether prior authorization is needed for the drug. Do this if a managed care plan manages your Medicaid.
- Ask your prescriber which GLP-1 drug is the preferred one; a preferred drug gets approved faster.
- Keep the approval or denial notice: it names the rule that covers you.
