Who qualifies
In the US, every GLP-1 sold needs a prescription. For an approved use listed on the label, a prescriber writes it:
- Blood sugar falls for adults with type 2 diabetes when this medicine is combined with diet and exercise.
- With type 2 diabetes and heart disease, adults who use it face a lower chance of a heart attack, stroke, or heart-related death.
- Among adults with chronic kidney disease and type 2 diabetes, it lowers the risk of death from heart problems, kidney failure, and worsening kidney disease.

Scam signals
- You need no prescription: a seller that skips it does not hold a US pharmacy license.
- A brand price far under what the manufacturer charges: for the brand, self-pay starts at $149 a month.
- Compounded drugs marketed as the brand: without FDA approval, none is Ozempic, Wegovy, Mounjaro or Zepbound.
- For salt forms, neither semaglutide sodium nor semaglutide acetate is the active ingredient in approved drugs.
- Misleading claims about matching the brand: FDA issued 30 warning letters to telehealth companies
- Harm reports: Compounded semaglutide is linked to 990 adverse event reports that FDA has.
- Leaving the drug itself out of monthly pricing was the issue in one program. The FTC took action over this; see the NextMed review.
