The prior authorization checklist
- Chart record: Your chart lists the weight condition your plan requires, or documents another approved Wegovy use, including heart risk and the liver disease MASH.
- Horizon NJ lists a weight rule for New Jersey: a BMI of 35 or above plus a condition linked to weight
- Records: The program you tried for diet and activity, along with your measured height and weight and weight-related conditions.
- Your prescriber files the prior authorization, not you.

Appeals after a denial
| Denial reason | What to send or ask |
|---|---|
| The plan excludes weight-loss drugs | This is a plan design choice. Find out from your employer's benefits team whether weight-loss drugs will be added at renewal. Blue Cross Massachusetts states that its exclusion cannot be appealed. |
| Criteria not met | Ask your prescriber to send the request again with your measured BMI and all weight-related conditions listed in your chart. |
| The drug is not on the plan's list | Ask the plan for a formulary exception, and find out which GLP-1 it prefers |
- Get the denial letter. It tells you which exact rule was not met.
- The appeal is filed by your prescriber, along with chart notes that answer that rule
- Should the plan's own appeal fail, ask an independent reviewer to conduct an external review.
