- Diagnosis: your chart must show the weight diagnosis the plan requested, or an approved Wegovy use for heart risk or MASH liver disease.
- A heart-risk use requires established heart disease and BMI 27 or higher
- The records need the diet and activity program you tried, your measured height and weight, and your weight-related conditions.
- Your prescriber is responsible for filing the prior authorization.
