Prior authorization checklist
- Diagnosis: Your chart lists the weight diagnosis requested by your plan, or heart risk and MASH liver disease.
- Lower weight rule: BMI 27 or higher with a weight-related condition
- Records: Your height and weight as measured, any weight-related conditions, and the diet and activity program you tried.
- Who submits it: Your prescriber files the prior authorization, not you.

Your appeal if you are denied
| Denial reason | What to send or ask |
|---|---|
| Criteria not met | Ask your prescriber to resubmit using your measured BMI (the rule requires a weight-related condition and BMI of 27 or more), along with every weight-related condition in your chart. |
| The drug is not on the plan's list | Request a formulary exception and ask which GLP-1 the plan prefers you use. |
- Get the denial letter because it identifies the exact rule you did not meet.
- Include chart notes that address the rule with your appeal. Your prescriber files it.
- If the plan's appeal does not succeed, request an outside review from an independent reviewer.
