- Diagnosis: your chart needs the weight diagnosis your plan wants, or another approved Wegovy use: heart risk, MASH liver disease.
- For renewal of coverage, 5% of baseline weight must be lost, per
- Records: your measured height and weight, the diet and activity program you tried, and any weight-related conditions.
- It is your prescriber who submits the prior authorization, not you.

- Your prescriber submits the appeal with chart notes that respond to the rule.
- After the plan rejects the appeal, ask an independent reviewer to conduct an external review.
