If you are denied: how to appeal
| Denial reason | What to send or ask |
|---|---|
| Prior authorization missing | Your prescriber submits the plan's form with your type 2 diabetes diagnosis and recent A1C results. |
| The plan wants another drug first | Ask the plan which drugs it wants you to try first, and provide records of your earlier attempts. |
| The drug is not on the plan's list | Ask for a formulary exception, and find out which GLP-1 the plan would rather you use. |
- Get the notice that explains the denial from your Medicaid plan.
- Your prescriber has the option to send the prior authorization again with chart notes attached.
- If the plan continues to deny it, you can ask for a state fair hearing.
