Prior authorization missingWhen your prescriber sends the plan's form, they include a recent A1C along with your diagnosis of type 2 diabetes.
The plan wants another drug firstAsk which drugs the plan would prefer you try first, and send records of medicines you have already tried.
The drug is not on the plan's listRequest a formulary exception from the plan, and ask which GLP-1 it prefers instead.
  1. The denial letter states the exact rule that was not met, so get that letter.
  2. Your prescriber files the appeal, and it includes chart notes that address that rule.
  3. Should the plan's appeal not succeed, ask an independent reviewer to conduct an external review.
