Prior authorization checklist
- Your chart lists the weight diagnosis requested by your plan.
- BMI by itself: 35 or higher, with no condition needed
- Heart or kidney condition: BMI 30 or higher, with CKD 3a+, uncontrolled blood pressure, or heart failure
- Condition with a lower BMI: 27 or higher, plus a prior heart attack or stroke, prediabetes, or PAD
- Approval through prior authorization
- The records list conditions related to weight, height and weight measurements, and the diet and activity program you tried.
- Filing: Your prescriber, not you, sends the prior authorization.

If you are denied: how to appeal
| Denial reason | What to send or ask |
|---|---|
| Criteria not met | Ask that your prescriber resubmit the request with your measured BMI and every weight-related condition recorded in your chart. The rule is BMI of 35, or 27 with a weight-related condition. |
| The drug is not on the plan's list | Request a formulary exception, and find out which GLP-1 the plan prefers instead. |
- Ask your Part D plan for a coverage determination, or an exception if the drug is not on its list.
- If the answer is no, ask the plan for a redetermination. Your prescriber can provide a supporting statement.
- If that fails, the appeal goes to an independent reviewer outside the plan.
How states and plans differ
Medicare Advantage plans with drug coverage follow these same rules. For example, Humana charges a $50 Bridge copay for each 30-day supply.
Each Part D plan sets its own copay tiers and drug lists, so copays for diabetes use vary.
The Medicaid rules for your state also apply if you have both Medicare and Medicaid: Does Medicaid Cover Saxenda? Criteria, Copay, How to Appeal.
