Rybelsus: the basics
Its half-life is about 1 week. After 7 days, half is gone, and about 3% remains after five half-lives (35 days).
In PIONEER 1, people with type 2 diabetes lost 3.7 kg over 26 weeks, compared with 1.4 kg on placebo.
Monthly prices in US dollars include list prices and self-pay offers taken from the maker's price page.

Prior authorization checklist
- Your chart includes type 2 diabetes, or it notes heart risk, another approved use of Rybelsus.
- Approval for prior authorization
- Records include the diabetes drugs you have tried and A1C results from recent tests.
- Your prescriber files the prior authorization, not you.

The appeal: if you are denied
| Denial reason | What to send or ask |
|---|---|
| Prior authorization missing | Your prescriber sends the plan's form with your type 2 diabetes diagnosis and recent A1C. |
| The plan wants another drug first | Ask the plan which drugs it wants you to try first. Send records of the medicines you have tried before. |
| The drug is not on the plan's list | Request a formulary exception from the plan. Ask which GLP-1 it prefers instead. |
- Ask your Part D plan for a coverage determination, or for an exception if the drug is not on its list.
- If the plan says no, request a redetermination. Your prescriber can send a supporting statement.
- If that does not work, the appeal is sent to an independent reviewer outside the plan.
State and plan differences
These rules also cover Medicare Advantage plans with drug coverage. For example, Humana lists a $50 Bridge copay for a 30-day supply.
Each Part D plan establishes its own copay tiers and drug lists, so the copay for diabetes use differs between plans.
